This may include disclosing your blood test results to your nominated medical practitioner for the purpose of assessing your suitability for the Program; and for internal purposes such as procedural assessments, risk management, product and service reviews, staff training, accounting and billing.

Your Choices

You may gain access to personal information about you that we hold. You can instruct us to correct errors or delete the information we have about you.
You may opt-out of any further contact from us.

To protect your privacy and the privacy of others, we may have to verify that you are who you say you are before we can give you access to, or change, information about you.
